Oral Immunotherapy (OIT) for Food Allergies | MedicareWell

What is a Food Allergy?

Food allergy means the immune system reaction after eating a specific food. This can happen even after eating a small quantity of that food. In some individuals. the food allergy can result in a life-threatening reaction called anaphylaxis. It affects approximately 8% of children under five years of age and 4% of the adult population, globally. Food allergy is often confused with food intolerance, but they are two separate terms. They both are troublesome but the latter is less severe, as it doesn’t involve a person’s immune system.

Types of Food Allergy

There are the following kinds of food allergies.

IgE-mediated

It is the most common type of food allergy. In this type, the immune system triggers the production of an immunoglobulin E antibody. Furthermore, the symptoms appear within seconds or minutes after consumption of certain foods. This type poses a risk of anaphylaxis.

Non-IgE mediated

This type of allergic reaction is not due to immunoglobulin E. However, other cells in the immune system cause this allergic reaction. There is difficulty in the diagnosis of this kind of allergy, as the symptoms appear after several hours.

Combined IgE and non-IgE mediated

Some individuals might face symptoms of both kinds of food allergies.

Symptoms

oral immunology

The symptoms of food allergy vary among people. Some individuals suffer from less severe symptoms while for others the symptoms can be severe and life-threatening. One gets the symptoms after a few minutes to a few hours of consuming food an individual is allergic to. Some common symptoms of food allergy are:

  • Mouth itchiness.
  • Eczema.
  • Hives.
  • Swollen lips, tongue, and other body parts.
  • Difficulty in breathing.
  • Wheezing.
  • Abdominal pain.
  • Diarrhea
  • Nausea
  • Dizziness

Anaphylaxis

Some people might suffer from anaphylaxis. The symptoms associated with anaphylaxis are:

  • Airways tightening.
  • Difficulty breathing due to swelling and feeling a lump in the throat.
  • Rapid heartbeat.
  • Decrease in blood pressure range.
  • Dizziness.
  • Vomiting

In case of anaphylaxis, immediate treatment is necessary, because it could lead to death or coma.

Food Allergy Treatment

Some of the options for food allergy treatment are:

  • The primary option for treatment is avoiding food that causes an allergic reaction. For this, a person must read the labels carefully to check for the ingredients that he/she is allergic to.
  • Certain medications help treat food allergies such as antihistamines, epinephrine, etc. Antihistamines relieve mild symptoms of food allergy like runny nose. On the other hand, epinephrine treats severe allergic reactions like anaphylaxis.
  • Oral immunotherapy is another method of treatment.

Let’s discuss oral immunotherapy like allergy immunotherapy in detail.

What is meant by Oral Immunotherapy?

Oral immunotherapy for food allergies involves eating or drinking a small quantity of food and drink a person is allergic to, then slowly increasing the amount he/she can have. This process requires a period of some weeks to months. The quantity of food and drink and timeframe changes depending on a person’s diagnosis and what food or drink they are allergic to.Moreover, the person is given food in the form of liquid or powder, the quantity is measured carefully and then added to another drink or food like yogurt, etc.

Dose Escalation

The amount of drink or food will be increased daily or after a few days, assuming that a person is no longer allergic to this specific food. This procedure of increasing the quantity is known as “Dose escalation”.

Once an individual reaches the maximum tolerance amount without any allergic reaction, he/she must keep consuming this amount daily for some time. The period could be months to a year and sometimes more. Moreover, if a person skips some days of consuming this food regularly the treatment’s effect could be worn off. Still, there is a possibility that a person eats the food rarely but keeps the advantages.

What is the goal of Oral Immunotherapy?

The main purpose of oral immunotherapy is to increase an individual tolerance to certain food allergens. This reduces the severity of allergic reactions and also prevents them. The purpose of oral immunotherapy is, to train the immune system to decrease its sensitivity to certain allergens. This is attained by gradually exposing the person to an increasing quantity of the allergen. This treatment increases the person’s tolerance level to certain allergens, without causing any allergic reaction. Moreover, this allows the person to manage accidental exposure to these allergens. This helps improve life quality. Additionally, it aids individuals to be fearless against unintentional exposure to allergens thus allowing them to lead a normal and happy life.

How does Oral Immunotherapy Work?

Oral immunotherapy works by immune system allergy desensitization. Let’s discuss how oral immunotherapy works in detail.

Assessing a Food Allergy

Different diagnostic tests like skin and oral food challenge tests are done to confirm the diagnosis of specific food allergies.

Evaluating the Patient’s Health Condition

It is important to evaluate and assess the patient’s following features:

  • Medical History.
  • Compatibility with Oral immunotherapy.
  • Overall health.

The Process of Allergy Desensitization

It has three stages:

Starting Dose Escalation

Initially, the patient is given a small quantity of the allergen. This is done in clinics to ensure patient safety. Moreover, the patient is observed for a few hours for any adverse reaction.

Increase in Dose

After initial dose escalation, there is a slow increase in the patient’s dose. This can be done in a home setting along with clinical visits to escalate doses under supervision. Moreover, regular follow-ups with health care professionals are essential for adjustments of doses.

Maintenance Stage

After reaching the maximum tolerance dose, the patient continues to intake this dose every day. This is important for desensitization of the immune system.

Mode of Action

When a person undergoes OIT for allergies, their immune system handles these allergens well. Initiating with a small quantity of allergens and gradually increasing the quantity helps in immune system training. This procedure helps to not result in sudden allergic reactions. So, OIT therapy plays a role in training the immune system to decrease its sensitivity to allergens with time.

Safety Protocol

The initial dose and an increase in dose are done under the complete supervision of the healthcare provider. Medical supervision is essential for managing potential allergic reactions like anaphylaxis. The healthcare provider recommends epinephrine or other medication throughout the treatment to prevent any risk of allergic reaction.

What are the eligibility criteria for Oral Immunotherapy?

Typically, healthcare professionals base eligibility criteria on various factors to ensure the safety and efficiency of the treatment. Some of these factors are:

  • The healthcare professional determines the history of allergic reactions to certain food products like eggs. Additionally, healthcare professionals consider an individual eligible in case of positive test results of food allergy after diagnostic procedures.
  • Age is another factor identifying whether a person is eligible or not. Oral immunotherapy treatment is typically for all age groups, however, some protocols can only be applied to older individuals.
  • Before starting the treatment, the healthcare professional assesses the patient’s health condition.
  • Patients’ and their family member’s commitment is another feature that determines the eligibility criteria.

What are the Benefits of Oral Immunotherapy?

Some of the benefits of oral immunotherapy are as follows:

  • Oral immunotherapy increases the level of allergy tolerance.
  • It decreases an individual’s sensitivity.
  • Oral immunotherapy improves the patient’s quality of life.
  • Moreover, the patient attains long-term benefits.
  • Oral immunotherapy can be personalized based on the patient’s need.

What is the Scientific Example of Oral Immunotherapy?

Egg Allergy

It is the most common allergy among children and can persist till adulthood. This increases the anaphylaxis risk. In an article published in The Journal of Allergy and Clinical Immunology, the researchers explored an immunotherapy effect on children and young ones suffering from egg allergy. The study had 50 egg allergy patients. The researchers made two groups; a control group and a group treated with oral immunotherapy. They treated the OIT group with raw egg white powder.

On the other hand, the control group avoided eggs for six months before administering them with oral immunotherapy. The researchers included individuals with a history of egg allergy, immune system sensitization, and, moderate to severe reaction, in a controlled food challenge environment, to heated egg whites. They aimed to reach a daily consumption of 1g of the white part of an egg.

Collection of Blood Samples

Researchers collected blood samples from the patients to measure antibody levels and molecules of egg allergen at baseline and after three and eight months of oral immunotherapy.

Further Experimentation

After eight months, the researchers categorized children consuming 1g of white egg part and not developing any symptoms as desensitized.

On the other hand, the researcher categorized children consuming less than 1g of egg white without developing any symptoms as partially desensitized and researchers continued giving them an increased dose.

Furthermore, the researchers gave a heated egg white to symptomatic children. Once the children reached their targeted dose, they started consuming the third part of an egg daily. After three months, the researchers performed an oral egg challenge with half part of a half-boiled egg. The researchers then allowed the patient who passed the challenge to intake the heated form of half part of an egg. However, they did not increase the quantity of raw eggs. The researcher allowed them to consume heated egg products daily.

Study Results

After eight months of OIT,44% of children could consume eggs without any problem,46% could consume eggs without major issues, and 10% stopped the treatment. Additionally, one child’s energy after six months on a special diet was completely resolved. The authors concluded that children with high levels of antibodies and those sensitive to all four proteins of eggs were more likely to discontinue the treatment. After 18 months of oral immunotherapy, 88% of children could regularly consume eggs and 72% could eat the targeted egg quantity without issues. They concluded that oral immunotherapy with raw eggs is beneficial for many patients and they could consume eggs daily, whereas some got immunity after prolonged treatment.

What are the Risks Associated with Oral Immunotherapy?

Some of the risks associated with oral immunotherapy are as follows:

  • The individuals might face mild to severe allergic reactions such as hives, anaphylaxis, etc. An individual can suffer from anaphylaxis more probably during the phase of dose escalation.
  • There is a chance that an individual might face gastrointestinal issues like diarrhea, and vomiting.
  • Some individuals might not get the desired desensitization level.
  • If an individual is consuming certain medication, there is a possibility that it might interact with the patient’s oral immunotherapy.
  • There is a possibility that a patient might get sensitive to something new.

Why is Oral Immunotherapy Better than Other Treatment Options?

It has several advantages over various treatment options. Some of them are:

  • As OIT gradually desensitizes the immune system, this could lead to long-term tolerance.
  • Oral immunotherapy improves the quality of life, as it allows individuals to manage accidental exposure to allergens.
  • The successful OIT treatment allows the patient to consume their favorite food, which they are allergic to, in moderate quantity without worrying about allergic reactions.
  • There is clinical evidence suggesting the efficacy of this oral allergy treatment.

What are the Limitations Associated with Oral Immunotherapy?

It offers limitations despite its various advantages. Some of the limitations are listed below:

  • Patients must adhere to the strict routine of slowly increasing allergen doses and maintenance. This requires commitment.
  • One of the limitations of oral immunotherapy is some side effects. These side effects include allergic reactions and gastrointestinal issues.
  • Oral immune therapy might not work for certain allergens. It offers more efficiency and efficacy against allergens such as eggs.

What are the Future Advancements of Oral Immunotherapy?

Future advancements in oral immunotherapy aim to make it a more efficient and accessible treatment. The researchers aim to customize oral immunotherapy according to a person’s genetic and immune profile. Moreover, they are trying to expand their treatment for various allergens. The researchers are formulating ways to reduce side effects and adverse reaction risks. The combination of oral immunotherapy with other treatment options increases both efficacy and efficiency. There are ongoing efforts to create simplified protocols for oral immunotherapy, thus making it an easier option for patients.

Conclusion

In conclusion, food allergy is a condition in which a person can’t eat specific food because of the immune system response.
